Within the Hospital at Home unit, patients benefit from a minimum of two in-person nursing visits, along with a daily provider visit, which may be conducted virtually or in person. Advanced Practice Providers (APPs) play a crucial role in screening for eligible Hospital at Home patients within TGH’s inpatient and emergency room environments. The clinical responsibilities of APPs include, but are not limited to, daily patient rounding via telehealth (and may be required to perform in-person assessment at a patient’s home when clinically necessary), admissions, discharges, and consultations. The APP will contribute to enhancing patient care and operational efficiency. APPs are empowered to make autonomous medical decisions, allowing them to provide a comprehensive range of diagnostic and therapeutic services.%26nbsp;%3C/span%3E%3Cbr%3E%3Cbr%3E%3Cspan style=%22color%5C:black;font-family%5C:Arial, Helvetica, sans-serif;font-size%5C:16px;%22%3EThe APP role encompasses participation in quality and process improvement initiatives, ongoing program evaluation, and the development of strategies for continuous improvement. The acute care Hospital at Home APP will collaborate closely with their Supervising Physician and multidisciplinary teams, as well as the hospital-based consulting services. QUALIFICATIONS:

• Nationally board-certified Acute Care NP or Adult Gerontology Acute Care NP, or a Physician Assistant.
• DEA required or obtained after the orientation period.
• Inpatient Hospital care and Internal Medicine preferred.
• Safe driving record required to operate hospital at home vehicles.
• Demonstrates excellent communication, critical thinking, and leadership skills.
• Ability to work effectively in a team.
• Commitment to ongoing professional development and process improvement.
